Team Leader - Home and Community Care

As a Team Leader, you’ll play a vital role in supporting a passionate team delivering care that helps people live independently, with dignity and choice.
This is a great opportunity for someone who can step in with confidence, build strong relationships quickly and make a meaningful impact from day one.

What you’ll be doing
In this role, you will lead and support a team of Support Workers and Clinicians to deliver safe, high-quality, person-centred care in the community.
- Provide day-to-day leadership, guidance and support to your team
- Coordinate service delivery, including rostering, leave management and shift approvals
- Act as a key point of contact and escalation for operational matters
- Work collaboratively with workforce planning, clinical and sales teams
- Support performance and development through coaching and mentoring
- Monitor service quality and drive continuous improvement
- Foster a safe, inclusive and values-led team environment
What you’ll bring
- Experience leading or supervising large teams
- Strong communication and relationship skills
- Ability to manage competing priorities and rostering
- Confidence navigating change
- Passion for supporting vulnerable communities
- Knowledge of in-home aged care
- Strong problem-solving and conflict resolution skills
Essential requirements
- Certificate IV in a relevant field or equivalent experience
- Current Australian Driver’s Licence
Desirable
- Clinical qualification (Nursing or Allied Health) and AHPRA registration
