Care Finder Adviser - South East/Limestone Coast Location

We are seeking Care Finder Adviser to join our team. The role aims to deliver specialist and intensive assistance to help people understand and access aged care and connect with other relevant support. The successful applicant should have the skill and ability to build and leverage a local network to deliver care finder services in the Mallee Coorong and upper South East regions.
The Role:
- Undertake assertive outreach
- Support clients to interact with My Aged care so they can be assessed
- Assist them to find aged care and community supports and services
Essential Criteria:
- Certificate IV in case management or client care coordination or similar qualification
- Experience in assessment, referral and creation/coordination of human services
- Current Driver's licence (essential)
