Working alongside GPs, Aboriginal Health Workers, Allied Health Professionals and other clinical staff, you will contribute to chronic disease management initiatives, preventative health programs and coordinated care activities aimed at improving long-term health outcomes within the community.

To be eligible for this position you must:
- Be currently registered with AHPRA as a Registered Nurse
- Have experience in chronic disease management within primary healthcare, community health, general practice or a similar setting
- Demonstrate strong knowledge of chronic disease management, care planning and preventative health
- Possess excellent communication and relationship-building skills
- Be able to work autonomously whilst contributing to a multidisciplinary team
- Hold a current Queensland Driver's Licence
- Hold current First Aid and CPR certification
- Experience working within an Aboriginal Medical Service or Community Controlled Health Organisation
- Experience using Best Practice software
- Nurse Immuniser qualification
- Experience with Closing the Gap initiatives and health assessments
- Knowledge of Aboriginal and Torres Strait Islander health issues and culturally safe healthcare practices
