Care Partner Retirement Living
Resthaven has an opportunity for a Care Partner Retirement Living to join our Retirement Living Team.
The Care Partner Retirement Living will provide comprehensive coordination and individualized case management and planning in partnership with Support at Home individuals. The Care Partner Retirement Living will oversee and assess the services provided, monitoring outcomes against each individual’s service plan, with an aim to optimize a person’s quality of life and achievement of goals. The role is a full-time permanent position.
Resthaven has Retirement Living villages located across the Adelaide metropolitan area, and as part of the responsibilities of the role, the successful applicant will travel to Retirement Living sites to work with residents.
Core Responsibilities
Referral for Assessment:
- Provide residents and/or their representatives advice about available support options including government funded supports.
- Discuss funding and eligibility options available to the residents.
Support at Home Care Management:
- Assist approved residents to access Support at Home care packages.
- Ensure all practices meet Aged Care Quality and Safety Commission Standards with the provision of safe and quality care and support services.
- Demonstrate resilience and maintain composure under pressure, ensuring effective decision-making and problem-solving in managing difficult interactions.
Case Management:
- Monitor the effectiveness of service provision by ensuring consumers receive the care and support services they require to meet identified needs, goals, and preference.
- Ensure compliance with the scheduled Review process and conduct in partnership with consumer and/or representatives.
- Monitor consumer satisfaction with all care, support, and service provision.
- Ensure that all consumers care, and support is underpinned by Consumer Directed Care Principles.
- Deliver excellent customer services to consumers and their representatives.
Essential Criteria:
- Minimum 3 years’ experience in a case management role.
- Appropriately trained aged care workers with relevant experience, holding (or working towards) a relevant qualification such as a Diploma of Nursing; Certificate III in Individual Support (Ageing) or Health Services Assistance; Certificate IV in Aged Care, Disability, or Community Services; or a Diploma in Community Services (Case Management) or Ageing Studies and Services.
- Prior experience in community aged care management or business management.
- Understanding of Support at Home Care Package Management.
- Excellent knowledge of the home care industry and Aged Care Quality and Safety Standards.
- Working knowledge of Microsoft programs, care delivery software, and mobile technology.
- Current valid Australia driver’s licence.
