Customer Intake Partner Community Services
Help older people navigate their care options and access the right support to continue living independently at home.
Resthaven is seeking a Customer Intake Partner to join our Murray Malley Adelaide Hills Community Services team.
In this relationship-focused role, you'll guide new clients and their families through their journey with Resthaven - from understanding their individual needs, goals and funding through to establishing personalised services and successfully onboarding them into Community Services.
You'll manage a pipeline of prospective clients while working closely with Care Partners, multidisciplinary teams and other stakeholders to provide a seamless and positive experience.
You will:
- Facilitate the admission and onboarding of new clients through face-to-face and virtual appointments.
- Build relationships with clients and families to understand their needs, goals and preferences.
- Provide information and guidance on available funding, services and support options.
- Develop individual client budgets and coordinate services aligned with their goals.
- Manage a pipeline of prospective clients and work towards agreed intake and sales KPIs.
- Ensure a high-quality client experience throughout the onboarding process.
- Develop relationships with community referral networks and promote Resthaven Community Services.
- Collaborate with Care Partners, multidisciplinary teams and other stakeholders to ensure a smooth transition into services.
- Maintain accurate client information and documentation across relevant systems.
Essential Criteria:
- Experience in a person-centred, relationship-focused, sales or business development role.
- Strong communication, relationship management and negotiation skills.
- Experience managing a customer or sales pipeline and working towards KPIs.
- Well-developed decision-making and analytical skills.
- The ability to independently manage and prioritise a varied workload.
- Confidence adapting to changing funding, legislative and operational requirements.
- A current driver's licence.
Experience in community aged care, client intake, CRM systems, client budgeting or multidisciplinary environments will be highly regarded.
