Virtual Health Services Health Coach | Regional Queensland

Working alongside a supportive multidisciplinary team, you will play an important role in the delivery of a Virtual Health Services Program, supporting clients through telemonitoring, health education, care coordination and community-based health promotion.

Job details:
- $90,000 – $100,000 per year + superannuation, dependent on experience

You will support the delivery and coordination of Virtual Health Services across the regional service area.
Your responsibilities will include:
- Support clients to use telemonitoring equipment and technology
- Assist clients with recording their vital signs
- Assist clients to transmit health data and results using provided technology
- Monitor the Virtual Health Services Dashboard, including participant readings, alerts, tasks and clinical escalations
- Use clinical knowledge and judgement to assess participant data and determine appropriate follow-up actions
- Provide timely interventions within your scope of practice and escalate clinical concerns where required
- Coordinate and participate in multidisciplinary case conferencing with GPs and other healthcare professionals
- Contribute clinical insights and support the implementation and follow-up of agreed care plans
- Provide clinical education and health promotion activities
- Educate clients about their health conditions and support them to improve their health and wellbeing
- Conduct home visits, school visits and outreach activities where clinically appropriate
- Facilitate access to relevant services and supports through care coordination and referral pathways
- Participate in the planning, promotion, implementation and evaluation of health promotion and prevention activities
- Support community awareness activities, health events and targeted health campaigns
- Contribute to research, quality improvement, data collection, reporting and program evaluation
- Maintain accurate and timely clinical documentation.

To be successful in this role, you will have:
- Tertiary qualifications such as a Certificate, Diploma or Bachelor qualification in Nursing, Aboriginal Health Practice or Aboriginal Health Work
- First Aid and/or CPR certification
- A current Queensland C Class Open Driver Licence
- Current Drivers Authorisation is desirable
- Working with Children Suitability Notice (Blue Card) and/or Police Check
- Current professional registration where applicable, including AHPRA registration where required
- Ability to undertake ongoing training and professional development
- Strong communication and interpersonal skills
- Ability to work collaboratively within a multidisciplinary healthcare team
- Commitment to culturally safe, patient-centred and holistic healthcare
- Competence using computers and general office technology
