You will provide high-quality, culturally safe nursing care and help coordinate each client's healthcare journey across both the clinic and community.
Your responsibilities will include:
- Delivering evidence-based primary healthcare nursing
- Assessing, planning, implementing and evaluating individualised nursing care
- Coordinating multidisciplinary care plans
- Managing internal and external referrals, including specialist and allied health services
- Supporting clients with ongoing health management and education
- Working closely with Aboriginal Health Practitioners, GPs and other healthcare professionals
- Building strong and respectful relationships with clients, families and the wider community
To be considered, you will have:
- Current unrestricted AHPRA registration as a Registered Nurse
- Minimum 3 years post-graduate nursing experience
- Experience planning, developing, implementing and evaluating nursing care
- Strong communication and interpersonal skills
- Ability to communicate effectively and sensitively with Aboriginal people
- An understanding of the health issues and barriers impacting Aboriginal people and communities
- Excellent organisational and time-management skills
- Ability to work flexibly within a busy primary healthcare environment
